Transcripts

Phoenix Seminary students and alumni are entitled to receive transcripts of their completed course work if they have no financial obligations to the Seminary.

Please complete the Transcript Request Form and submit it to the Registrar along with appropriate payment. Include your full name and the address(es) to which you would like your transcripts mailed.

Transcript Request Form

There is a $5.00 charge per official transcript (payable by credit / debit card, check, or cash). If two transcripts are being mailed to a single address in a single envelope, this fee will only be charged once. This fee is waived for the first degree-posted transcript requested by a student following graduation, and will not be charged for transcripts issued to Ottawa University as part of the cooperative Master of Arts in Professional Counseling program.

Upon the receipt of your signed transcript request and payment, an official transcript will be issued to appropriate institutions or individuals in a sealed, signed envelope. A confirmation will also be sent the email address listed on the request.

You may fax your Transcript Request Form to 602-850-8080, or scan your signed and completed form and email it to registrar@ps.edu. You may also mail your request to:
