Forms & Documents

Below you will find an array of forms, documents, and other resources that will be useful to you throughout your time of study at Phoenix Seminary. Each resource is categorized by area, and some resources will therefore appear in more than one category.

Academic Forms

  • Academic Petition — Use this form to request an exemption from standard academic policies. For example, to request to take a course as an independent study, or to create a new course as an individualized study.
  • Add/Drop Form — Use this form to to add individual courses OR drop or withdraw from individual courses after Registration Deadline.
  • Change of Degree Petition — Submit this petition to request a change from one program or concentration to another program or concentration.
  • FERPA Form — Students may complete this form if they would like their educational information shared with another party such as a parent or spouse. Please email the completed form to the Registrar’s Office.
  • LOA-Withdrawal Form — Students who will not be registering for courses in a semester must complete the Leave of Absence / Withdrawal Form. If a student desires to withdraw from the Seminary with no intentions of returning, the student must complete the LOA / Withdrawal Form. The student should schedule an appointment with the Dean of Students and / or the Registrar at the time of submission of the form to ensure that all matters are in proper order.
  • Transcript Request Form — There is a $5.00 charge per official transcript (payable by credit / debit card, check, or money order). An official transcript will be issued to appropriate institutions or individuals in a sealed, signed envelope within ten business days of the receipt of your signed transcript request and payment.

Student Billing

  • Spring 2017 Fee Schedule — This important document explains tuition and fees for each semester, as well as dates for adding or dropping classes (along with the refund schedule).

  • Spring 2017 Tuition Installment Plan Authorization Form — This important document is completed by all students who wish to fulfill their tuition payment obligations in automatic installments.

Grades and Transcripts

An official transcript will be issued to appropriate institutions or individuals in a sealed, signed envelope within ten business days of the receipt of your signed transcript request and payment.
